HIPAA Notice of Privacy Practices
This Notice describes how protected health information ("PHI") may be used and disclosed and how you can access your PHI, when HIPAA applies to information we handle.
1. Scope
This Notice applies only to PHI maintained by Body Observability when we are acting in a role that makes HIPAA applicable to the information we handle (for example, under a business associate agreement). Other entities involved in providing services may have separate notices.
2. Our responsibilities
- Maintain the privacy and security of PHI.
- Provide this Notice of our legal duties and privacy practices.
- Notify you following a breach of unsecured PHI as required by law.
3. Uses and disclosures
When permitted, we may use and disclose PHI to provide services you request, support longitudinal reporting, coordinate with partners supporting requested services, perform operations and quality improvement, and comply with law.
4. Your rights
- Request access to and copies of your PHI.
- Request corrections to PHI you believe is incorrect or incomplete.
- Request restrictions on certain uses and disclosures (we may not be required to agree).
- Request confidential communications.
- Request an accounting of certain disclosures, as permitted by law.
